Welcome to our SPX Faith Formation Program! Our Parish offers a personalized, developmentally appropriate program designed to foster in each child a deep and personal relationship and encounter with God and our Catholic faith. Here, you will find helpful information and resources to join and stay informed.
FOR NEW CHILDREN/PARENTS:- Learn about our Faith Formation Program for Grades 1-8 in our brochure .
- To register your child for our Faith Formation Program for Grades 1-8: Faith Formation Registration.
- If you would like to volunteer, complete the SPX Volunteer Form below and go to the "Welcome" tab on the website and click "Protecting God's Children" and follow the procedure listed. Any questions, email Kathy G. in the office [email protected].
- Complete the SPX Parent Volunteer Form
- Complete and scan back the permission to take a photo/video release form to Kathy G. in the [email protected]: Photo / Video Release Form.
- To register your child for Component II of our Program: Focus Experiences, please view our Focus Experience Schedule 24-25 and send an email to [email protected]. They will register you.
FOR CURRENT CHILDREN/PARENTS:- To make a Focus Experience change: Select the appropriate form by your child's grade and complete: Focus Experience Registration GRADES 1-4 or Focus Experience Registration GRADES 5-8.

- Sponsor Form: Baptism/Confirmation Sponsor Form
